Q1. Explain
interpersonal psychotherapy in terms of its characteristics and techniques.
Interpersonal
Psychotherapy (IPT) is a time-limited, evidence-based treatment that focuses on
interpersonal issues, which are posited to play a significant role in the onset
and maintenance of psychological disorders. The primary goal of IPT is to
improve interpersonal functioning, which in turn can alleviate psychological
symptoms. It was originally developed to treat major depressive disorder but
has since been adapted to treat a range of other disorders.
Characteristics of Interpersonal Psychotherapy:
Time-limited:
Typically, IPT is administered over 12-16 sessions, although the number can
vary based on the individual's needs and the specific setting.
Here-and-Now
Focus: While past relationships might be discussed to provide context, the
primary focus of IPT is on current interpersonal relationships.
Medical Model:
IPT often employs a medical model, in which the psychological disorder is
framed as an illness. This can be helpful in reducing blame or shame associated
with the disorder.
Symptom-oriented:
The therapy seeks to reduce symptoms and improve interpersonal functioning.
Structured: IPT
follows a specific structure that includes an initial phase (assessment and
identification of the interpersonal focal area), a middle phase (addressing the
interpersonal issues), and a termination phase (consolidating gains and
preparing for the end of therapy).
Techniques and Main Components:
Interpersonal
Inventory: This is an assessment of the patient's significant relationships and
patterns within those relationships. It helps identify which of the following
problem areas to focus on.
Identification
of Interpersonal Problem Areas: IPT posits that there are typically four main
problem areas that contribute to psychological distress:
Grief: Relating
to complicated bereavement after the death of a loved one.
Role Disputes:
Struggles or conflicts with a significant person in one's life.
Role
Transitions: Difficulty in adapting to a major life change such as becoming a
parent, retiring, or divorcing.
Interpersonal
Deficits: Refers to long-standing difficulties in forming and maintaining
healthy relationships.
Communication
Analysis: In cases where communication issues are evident, the therapist might
analyze a recent interaction to identify and modify unhelpful communication
patterns.
Expressive
Techniques: These encourage the patient to explore and express feelings about
interpersonal incidents, helping them understand and process their emotions.
Clarification:
The therapist may help the patient clarify and understand their feelings,
thoughts, and behaviors in interpersonal situations.
Encouragement
of Affect: This involves helping the patient to express and accept their
emotions.
Use of
Therapeutic Relationship: The relationship between therapist and patient can be
utilized as a model or example of healthy interpersonal interaction.
Decision
Analysis: If a patient is facing an important decision, the therapist can help
them weigh the pros and cons related to it, ensuring the choice aligns with
their goals and values.
Problem-solving:
Helps the patient develop strategies to address interpersonal issues.
IPT has been
shown to be effective in numerous studies and is considered a first-line
treatment for conditions like major depressive disorder. Over time, it's been
adapted to treat other conditions, including bulimia nervosa, bipolar disorder,
and posttraumatic stress disorder, among others.
