1. A 68 year old person has come to causality with acute retrosternal pain, severe breathlessness and profuse sweating of half-an-hour duration. List the differential diagnosis. How would you investigate and manage this case?

2. A sixty year old female has come with a lump in right breast. On examination there is a blood stained discharge from her nipple and a nodule in right axilla. What is your differential diagnosis? How would you investigate and manage this case?

3. A 62 year diabetic woman presents to her family physician with a 10 year history of diabetes mellitus. Earlier she was on oral agents, now she is taking insulin. Patient is DOE, has no chest pain for 3 months, she is a known Hypertensive and is on HCTZ. On examination her BP was 160/90 mmHg, BMI was 42 and Fundus showed non proliferative retinopathy. Investigations reveal fasting blood glucose was 243 mg/dl. ECG shows LVH with strain pattern. Urine sample is positive for microalbuminuria. What advice you will give to the patient? What will be the treatment of HT? Describe the role of microalbuminuria.
